For Allied Health professionals and participants:
NDIS How do you get funding for an Assistance Animal in your plan?
I need a report for NDIS to fund my Assistance Dog.
Go to THIS LINK & scroll down to ‘Further information and Resources. Click on ‘Assistance Animals Assessment Template’. This is what will need to be submitted. This template is completed by an OT (who you may employ solely for this purpose), Paws for Assistance Dog Training (PADT) and the NDIS participant where possible. It is a long process for all involved.
PADT is unable to assist with this report unless the participant has signed up and begun training under PADT.
PADT charges by the hour for this report.
