Collection of personal information
* Confidential client information may include but not limited to:
medical physical and or psychiatric history, test results, conversations, financial information, drivers license or photographic identification, text or computer messages and emails, verbal information.
* The Trainer ensures that the Client’s personal files will remain private and confidential and will not be disclosed unless the Client has provided the Trainer with written permission.
The information collected is destroyed or permanently de-identified when it is no longer needed for any purpose
* The Trainer will act as an Advocate for the Client should the need arise and the Client must provide written permission before this occurs.
Cameras and recording devices in the workplace
Cameras and recording devices may only be used with the permission of the Client and in the context of provision of services.
Recording may enhance the Client’s learning experience within the session context.
PADT will never use any recording without the client’s written consent.
Paws for Assistance Dog Training is committed to protecting the privacy of every client.
Information is collected solely for the purpose of safety of the client, training and handling within the sessions. This information will not be passed on to any 3rd party unless required by law and with the Client’s consent.
The Trainer makes every effort to securely store and protect the supplied information. Once this information is no longer required, it will be destroyed or de-identified to the best of the Trainer’s knowledge.
Privacy for the Trainer
Unless permission has been granted by the Trainer, the Trainer prohibits the client from using the Trainer’s own name or business name on any social media site including but not limited to Facebook, Instagram, Twitter, personal blogs, micro blogs, wikis, message boards, chat rooms, electronic newsletters, online forums and any other social networking sites. Ask the trainer first!
